Transitional Housing

Transitional Housing allows a young person to live independently over their last months of care.

Goal:

  • Transition To Adulthood

Beliefs:

  • Birth family and friends are the youth's best resources.
  • Teens exiting the system need to make their own decisions.
  • Experiencing consequences is vital to learning.

Referrals:

  • 17 or 18 years of age
  • Currently in foster or group care
  • Support of:
    • Foster parents or group home staff
    • County Social Worker and County ILP Worker and Coordinator

Mentor:

  • Non-system adult to provide support
  • Selected by the young adult

Transitional Housing Team:

  • Young adult and family members
  • CSD Worker and ILP Worker
  • Youth & Family Social Worker and Licensing Coordinator
  • Friends and Mentors
