Supporting Youth Transitioning to Independence
Youth & Family Programs’ Transitional Housing program provides safe, stable housing and supportive services for young adults preparing for independent living. Serving youth across Shasta, Tehama, and Butte Counties, this program is designed to help participants build the skills, confidence, and stability needed for long-term success.
A Foundation for Independence
Transitioning out of foster care can be challenging. Our program offers a structured and supportive environment where young adults can focus on personal growth and future planning.
Participants receive guidance in:
Housing stability
Employment readiness
Financial literacy
Education planning
Daily life skills
Safe and Supportive Housing
Providing stable housing is a key component of the program. Participants live in environments that promote responsibility, accountability, and independence while still offering support when needed.
Personalized Support and Goal Setting
Each participant works with a dedicated case manager to develop a personalized plan based on their goals and needs.
Support includes:
One-on-one case management
Goal setting and progress tracking
Resource connection and referrals
Ongoing mentorship and encouragement
Building Life Skills for Long-Term Success
